Under section 64 of the Freedom of Information Act 1982 (Vic) (the Act), the Victorian Information Commissioner is required to report annually to Parliament on the operation of the Act for all agencies and Ministers. This reporting forms part of OVIC's Annual Report which is usually tabled during Parliament’s Spring sitting.
Under section 64B of the Act, all agencies and Ministers are required to provide requested information or data to the Information Commissioner for this reporting purpose. Your co-operation is sought in completing this electronic survey regarding FOI matters handled for the period of 1 July 2025 to 30 June 2026. Please submit your survey by no later than 5:00pm on 29 July 2026.
This survey collects certain personal information, including staff names, position titles and contact details. Please click HERE for our personal information notice of collection. We ask that if you include names, position titles and/or contact information for individuals other than yourself into the survey, that you also provide them with a copy of this notice of collection.
When completing the survey, please do not enter any personal information into the free text fields.
On each of the following screens you will be asked to enter data required for your return into the fields provided. The survey validates the data entered as it is completed, avoiding the need to clarify inconsistencies after your return is submitted. This means that some of the screens will not allow you to continue until you have met certain requirements. For example, the total of personal and non-personal requests received must be equal to the total number of requests received at Step 7 before you can progress to Step 8.
At any point during the completion of your return you can save your progress by clicking on the [Save] button at the bottom of the screen. This allows you to exit the return, by clicking the [Exit] button, without losing the data you have entered therefore, your return can be completed over several sessions.
The ability to save partially completed returns means that you can start entering your list of exemptions at Step 10 or decision makers at Step 11 and save your data to allow you to return to complete your input at a later stage.
It is suggested that you save the link to this application to your internet favourites to allow you to return to it easily.
Individual screens can be printed by clicking the [Print] button, although large amounts of text may not print via this method. The [Summary Report] button will provide a print preview of all information entered (including all text).
Clicking the [Next] button will validate the data in the current screen and advance to the next screen. Clicking the [Back] button will return to the previous screen and allow you to change the data previously entered. You should ensure that you [Save] your data before going [Back] as it will not be automatically saved.
Please note that this application will time out automatically after two hours if no data has been entered.
A PDF document detailing and explaining the information required to complete this return can be accessed by clicking on the Report Overview.
Please enter your agency username (email address) and password, then click on the Start button to log-in and begin your return. If you do not have a username(email address)/password, please contact us.
